Fluvastatin + fenofibrate is a combination therapy used to treat severe primary hypercholesterolemia and mixed dyslipidemia. Fluvastatin is a statin that selectively and competitively inhibits the hepatic enzyme HMG-CoA reductase (3-hydroxy-3-methylglutaryl-coenzyme A reductase), the rate-limiting step in cholesterol biosynthesis. This results in decreased plasma total and LDL cholesterol levels. Fenofibrate is a fibrate that activates peroxisome proliferator-activated receptor alpha (PPARα), increasing lipolysis via activation of lipoprotein lipase and reducing apoprotein C-III. This leads to enhanced catabolism of triglyceride-rich particles and reduced secretion of VLDL. The combination has been shown to provide substantial improvement in lowering LDL cholesterol and triglycerides compared with monotherapy[3][5][6][8]. Both drugs are primarily indicated for lipid disorders; their combined use may increase risk of side effects such as liver damage or myopathy[1].
