Folate + cobalamin is a combination of vitamin B9 (folate/folic acid) and vitamin B12 (cobalamin) being investigated as an adjunctive treatment for schizophrenia. Developed by researchers at Massachusetts General Hospital, this combination targets the one-carbon metabolism pathway to address metabolic imbalances associated with residual psychiatric symptoms. By providing essential cofactors for the conversion of homocysteine to methionine, the therapy aims to enhance DNA methylation and reduce homocysteine levels, which are frequently elevated in patients with schizophrenia. The inclusion of cobalamin (B12) is specifically intended to potentiate the downstream effects of folate on methylation reactions and to mitigate the risk of masking a B12 deficiency (pernicious anemia). This nutritional supplement intervention has been evaluated in a Phase 4 randomized, double-blind, placebo-controlled trial in Ethiopia, focusing on adults with stable residual positive or negative symptoms despite standard antipsychotic treatment.
