FOLFIRI + celecoxib is a combination regimen used in the treatment of advanced colorectal cancer. FOLFIRI consists of irinotecan, leucovorin (folinic acid), and fluorouracil (5-FU), which together act as cytotoxic chemotherapy agents targeting rapidly dividing tumor cells. Celecoxib is a selective cyclooxygenase-2 (COX-2) inhibitor that has anti-inflammatory properties and may exert antitumor effects by inhibiting COX-2-mediated pathways involved in tumor growth, angiogenesis, and resistance to apoptosis. The rationale for combining these agents is to potentially enhance antitumor efficacy through both direct cytotoxicity (from FOLFIRI) and modulation of the tumor microenvironment or inflammation-related pathways (via COX-2 inhibition with celecoxib). Clinical studies have shown that this combination is generally well tolerated; however, the addition of celecoxib to FOLFIRI has not demonstrated significant improvement in response rates or survival outcomes compared to FOLFIRI alone[1][3]. The regimen remains investigational for this indication.
