FOLFIRINOX + IPI-926 is an investigational combination therapy studied primarily for advanced pancreatic cancer. FOLFIRINOX is a chemotherapy regimen consisting of four drugs: 5-fluorouracil, leucovorin (folinic acid), irinotecan, and oxaliplatin. These agents work together to destroy rapidly dividing cancer cells by interfering with DNA synthesis and cell division[3][5][6]. IPI-926 (Saridegib) is an oral small molecule inhibitor targeting the Hedgehog signaling pathway via inhibition of the Smoothened (Smo) protein[1][2][8]. Preclinical studies suggested that Hedgehog pathway inhibition could deplete tumor-associated stroma, potentially improving chemotherapy delivery in pancreatic tumors[1][2]. In phase Ib clinical trials, this combination showed antitumor activity but also notable toxicity; further development was halted after negative results in related studies combining Hedgehog inhibitors with other chemotherapies[2].
