FOLFOX + FOLFIRI + panitumumab

FOLFOX + FOLFIRI + panitumumab is a combination treatment regimen evaluated in the CAIRO5 phase 3 clinical trial, conducted by the Dutch Colorectal Cancer Group (DCCG). This regimen is designed for patients with initially unresectable, liver-only colorectal metastases, specifically those with left-sided tumors and RAS/BRAF wild-type status. It consists of a doublet chemotherapy backbone—either FOLFOX (oxaliplatin, leucovorin, and 5-fluorouracil) or FOLFIRI (irinotecan, leucovorin, and 5-fluorouracil)—administered alongside panitumumab (Vectibix). Panitumumab is a fully human monoclonal antibody that targets the epidermal growth factor receptor (EGFR), inhibiting the signaling pathways that drive tumor proliferation. The goal of this intensive induction therapy is to downsize liver metastases to enable surgical resection. Results from the CAIRO5 trial indicated that while the addition of panitumumab increased response rates compared to bevacizumab, it did not improve progression-free survival and was associated with higher toxicity.
