FOLFOXIRI + QL1706 + bevacizumab is an investigational combination chemo-immunotherapy regimen being evaluated for the first-line treatment of metastatic colorectal cancer (mCRC). This intensive regimen combines the FOLFOXIRI chemotherapy backbone (irinotecan, oxaliplatin, leucovorin, and 5-fluorouracil) with the anti-angiogenic monoclonal antibody bevacizumab and the novel bifunctional antibody QL1706. QL1706, developed by Qilu Pharmaceutical using its MabPair technology, is a single bifunctional antibody that simultaneously targets programmed cell death protein 1 (PD-1) and cytotoxic T-lymphocyte-associated protein 4 (CTLA-4). The combination is designed to maximize anti-tumor activity by integrating cytotoxic chemotherapy, vascular normalization through VEGF inhibition, and dual immune checkpoint blockade. It is currently being studied in a Phase II trial led by Fujian Cancer Hospital specifically for patients with RAS/BRAF wild-type, left-sided metastatic colorectal cancer.
