This is an investigational combination regimen consisting of five agents: - **Folinic acid (leucovorin):** A reduced form of folic acid that enhances the effects of fluorouracil. - **Fluorouracil (5-FU):** An antimetabolite chemotherapy that inhibits thymidylate synthase, disrupting DNA synthesis in rapidly dividing cells. - **Oxaliplatin:** A platinum-based chemotherapeutic agent causing DNA crosslinking and apoptosis. - **Bevacizumab:** A monoclonal antibody targeting vascular endothelial growth factor A (VEGF-A), inhibiting angiogenesis. - **Durvalumab:** An immune checkpoint inhibitor targeting programmed death-ligand 1 (PD-L1), enhancing T-cell mediated anti-tumor response. - **Oleclumab:** A monoclonal antibody targeting CD73, which reduces adenosine-mediated immunosuppression in the tumor microenvironment. This combination has been studied for metastatic microsatellite-stable colorectal cancer. The rationale is to combine standard cytotoxic chemotherapy and antiangiogenic therapy with dual immunotherapy to potentially enhance anti-tumor efficacy. In clinical trials, this regimen showed a manageable safety profile but did not demonstrate sufficient efficacy to warrant further development for this indication[6][8].
