fosmanogepix + standard of care antifungal agent

Fosmanogepix (APX001) is a first-in-class, broad-spectrum antifungal prodrug that is rapidly converted by systemic phosphatases to its active moiety, manogepix. Manogepix inhibits the fungal enzyme Gwt1 (GPI-anchored protein biosynthetic palmitoyltransferase), which is essential for the glycosylphosphatidylinositol (GPI) anchor biosynthesis pathway. This inhibition prevents the maturation and localization of GPI-anchored proteins, which are critical for fungal cell wall integrity, adhesion, and biofilm formation. Developed by Basilea Pharmaceutica (following acquisition from Amplyx Pharmaceuticals, a Pfizer affiliate), fosmanogepix is being investigated for the treatment of invasive fungal infections caused by Candida, Aspergillus, and various rare molds. In clinical trials, it is often evaluated in combination with or compared to standard-of-care antifungal agents, particularly in high-risk populations such as patients with acute myeloid leukemia (AML) and neutropenia.
