Fumaric acid esters + apremilast is a combination of two oral small molecule drugs used in the management of moderate-to-severe plaque psoriasis and psoriatic arthritis. Fumaric acid esters, primarily dimethyl fumarate and related salts, are immunomodulators that exert their effects through antioxidant and anti-inflammatory pathways, notably by activating the Nrf2 signaling pathway and inhibiting NF-κB, leading to reduced oxidative stress and inflammation. Apremilast is a phosphodiesterase 4 (PDE4) inhibitor that increases intracellular cAMP levels in immune cells, resulting in decreased production of pro-inflammatory cytokines such as TNF-α, IL-12, IL-23, IFN-γ and increased anti-inflammatory mediators like IL-10. Both agents are approved for use as monotherapy or in combination with other systemic therapies for psoriasis; however, their combined use specifically as a fixed-dose or co-packaged product does not appear to be formally approved but may be considered off-label or investigational for patients with inadequate response to monotherapy[1][5][8][10]. The rationale for combining these agents lies in their complementary mechanisms targeting different inflammatory pathways implicated in psoriasis.
