Furmonertinib + pemetrexed is a combination therapy used primarily in the treatment of non-small cell lung cancer (NSCLC), particularly in patients with epidermal growth factor receptor (EGFR) mutations who have developed leptomeningeal metastases or have progressed after prior EGFR tyrosine kinase inhibitor (TKI) therapy. Furmonertinib is a third-generation, small molecule EGFR TKI that irreversibly inhibits mutant forms of the EGFR, including those with resistance mutations such as T790M and exon 20 insertions. Pemetrexed is an antifolate chemotherapy agent that disrupts folate-dependent metabolic processes essential for cell replication. This combination has been investigated both systemically and with intrathecal administration of pemetrexed to target central nervous system involvement. Clinical studies suggest this regimen can provide intracranial symptom relief and disease control in heavily pretreated NSCLC patients[2][9][7].
