Overview

Furosemide + sodium chloride is an investigational drug combination used as a pharmacological challenge in clinical research, specifically in a Yale University pilot study (NCT00611741). The regimen consists of furosemide, a loop diuretic that inhibits the Na-K-2Cl cotransporter (NKCC1/NKCC2) in the kidney, administered alongside high-dose sodium supplements (typically 8g per day in divided doses). This combination is employed to manipulate plasma osmolality and observe downstream effects on brain glutamate and GABA concentrations, as measured by MR spectroscopy (MRS). The sodium supplementation serves to maintain electrolyte balance and prevent severe hyponatremia during the diuresis induced by furosemide. This research is primarily focused on investigating the link between osmotic regulation and cortical glutamate levels in healthy volunteers and patients with schizophrenia.
