Fuzheng Kang'ai Decoction is a traditional Chinese herbal medicine formula widely used as an adjunctive therapy for non-small cell lung cancer (NSCLC) in China. It is composed of multiple herbs and has been shown to inhibit tumor growth, metastasis, and induce apoptosis in NSCLC cells. Mechanistically, it acts through several pathways: - Induces ferroptosis by suppressing glutathione peroxidase 4 (GPX4) via the system xc−/GSH/GPX4 axis. - Activates AMP-activated protein kinase alpha (AMPKα), leading to increased expression of insulin-like growth factor binding protein 1 (IGFBP1) and forkhead homeobox type O3a (FOXO3a). - Promotes apoptosis through activation of caspase-3 and deregulation of Bcl-2 family proteins via the STAT3/Bcl-2/caspase-3 pathway. - Inhibits metastasis by downregulating matrix metalloproteinase 9 (MMP9) through suppression of signal transducer and activator of transcription 3 (STAT3), also affecting epithelial-mesenchymal transition markers such as N-cadherin and vimentin. FZKA has been studied both in vitro and in vivo, including clinical trials as an adjuvant with EGFR tyrosine kinase inhibitors or immune checkpoint inhibitors for advanced NSCLC[1][2][4][5][6].
