Gabapentin + milnacipran is a combination of two small molecule drugs, each with distinct mechanisms of action. Gabapentin is an anticonvulsant and analgesic that modulates the activity of voltage-gated calcium channels in the central nervous system, reducing excitatory neurotransmitter release and providing relief from neuropathic pain. Milnacipran is a selective serotonin-norepinephrine reuptake inhibitor (SNRI) that increases levels of serotonin and norepinephrine in the brain by inhibiting their reuptake, which can help regulate pain signaling and mood. This combination may be considered for conditions such as fibromyalgia or neuropathic pain where both central sensitization and mood dysregulation are involved; however, there are no approved fixed-dose products combining these agents, and clinical data on their combined use are limited to case reports or off-label practice[1][2][3]. Both drugs have independent FDA approvals for specific indications but not as a fixed combination.
