gabapentin + nortriptyline is a fixed-dose combination pharmaceutical product developed by Sanix Formulation, primarily indicated for the management of neuropathic pain. This combination leverages the synergistic effects of two distinct classes of medications: gabapentin, a gabapentinoid, and nortriptyline, a tricyclic antidepressant (TCA). Gabapentin acts by binding to the alpha-2-delta (α2δ) subunit of voltage-gated calcium channels in the central nervous system, which reduces the influx of calcium into nerve terminals and subsequently inhibits the release of excitatory neurotransmitters such as glutamate and substance P. Nortriptyline primarily functions as a norepinephrine reuptake inhibitor and, to a lesser extent, a serotonin reuptake inhibitor, thereby enhancing the descending inhibitory pain pathways in the spinal cord. Together, these mechanisms provide a multi-modal approach to pain relief, often proving more effective than monotherapy for conditions like postherpetic neuralgia and diabetic peripheral neuropathy.
