Galunisertib + capecitabine is a combination therapy being investigated for the treatment of advanced chemotherapy-resistant colorectal cancer, particularly in patients with peritoneal metastases. Galunisertib is an oral small molecule TGFβR1 inhibitor that targets the TGF-β signaling pathway, which is implicated in drug resistance in colorectal cancer. Capecitabine is an oral fluoropyrimidine chemotherapy agent that is converted to 5-fluorouracil in the body. The combination aims to overcome resistance to 5-FU-based chemotherapy, especially in the mesenchymal subtype of colorectal cancer (CMS4) which has elevated TGF-β signaling causing drug resistance against multiple anticancer drugs.
