gemcitabine + albumin-bound paclitaxel + adebrelimab + liposomal irinotecan + oxaliplatin + 5-fluorouracil + leucovorin

This sequential chemo-immunotherapy regimen is being investigated as a conversion therapy for patients with borderline resectable or locally advanced pancreatic cancer. The treatment protocol consists of two distinct phases: an initial induction phase with gemcitabine and albumin-bound paclitaxel (AG) combined with the PD-L1 inhibitor adebrelimab, followed by a consolidation phase using a modified FOLFIRINOX backbone (liposomal irinotecan, oxaliplatin, 5-fluorouracil, and leucovorin) also in combination with adebrelimab. This strategy aims to maximize tumor shrinkage and increase the rate of radical surgical resection (R0 resection) by alternating non-cross-resistant chemotherapy backbones while maintaining continuous immune checkpoint inhibition. The regimen is currently under evaluation in a Phase II clinical trial sponsored by Shanghai Zhongshan Hospital.
