This combination regimen consists of the cytotoxic agents gemcitabine and albumin-bound paclitaxel (nab-paclitaxel) paired with a monoclonal antibody targeting the PD-1 checkpoint. Gemcitabine is a nucleoside analog that inhibits DNA synthesis by targeting ribonucleoside reductase and incorporating into DNA strands. Albumin-bound paclitaxel is a microtubule inhibitor that induces mitotic arrest and cell death. The PD-1 inhibitor component blocks the interaction between the Programmed Cell Death Protein 1 (PD-1) receptor on T cells and its ligands (PD-L1/PD-L2) on tumor cells, thereby reversing immune suppression and enhancing the anti-tumor immune response. This specific chemo-immunotherapy combination is being evaluated as a first-line treatment for locally advanced pancreatic cancer, often administered in sequence with stereotactic body radiotherapy (SBRT) to improve local control and systemic outcomes.
