The combination of gemcitabine, asparaginase, ifosfamide, dexamethasone, and etoposide, commonly known as the **GELIE** regimen, is an intensive multi-agent chemotherapy protocol developed primarily by researchers at **West China Hospital** (Sichuan University). This regimen is specifically designed for the treatment of aggressive natural killer (NK)/T-cell malignancies, such as extranodal NK/T-cell lymphoma (nasal type) and aggressive NK-cell leukemia. These diseases are often resistant to traditional anthracycline-based therapies (like CHOP) due to the high expression of P-glycoprotein (multidrug resistance). The GELIE regimen circumvents this resistance by utilizing agents that are not substrates for the multidrug resistance (MDR) pump, such as gemcitabine and L-asparaginase. The therapeutic efficacy is achieved through a synergistic multi-targeted approach involving DNA synthesis inhibition, DNA alkylation, topoisomerase II interference, and the depletion of L-asparagine, which is essential for the survival of lymphoma cells lacking asparagine synthetase.
