Overview

gemcitabine + cisplatin + ipilimumab is an investigational combination therapy regimen evaluated as a first-line treatment for patients with metastatic urothelial carcinoma. This regimen consists of the standard-of-care chemotherapy doublet, gemcitabine and cisplatin, supplemented with the immune checkpoint inhibitor ipilimumab. Ipilimumab is a recombinant human monoclonal antibody that binds to the cytotoxic T-lymphocyte-associated protein 4 (CTLA-4), an immune checkpoint molecule that downregulates T-cell activation. By blocking CTLA-4, ipilimumab promotes an active immune response against tumor cells. Gemcitabine, a nucleoside analog, and cisplatin, a platinum-based alkylating agent, provide direct cytotoxic effects by interfering with DNA synthesis and repair. The combination was evaluated in the Phase II Hoosier Cancer Research Network GU10-148 trial; while it showed a high objective response rate, it did not meet the primary endpoint of improved 1-year overall survival.
