Overview

This combination therapy consists of the chemotherapy agents gemcitabine and cisplatin paired with the EGFR-targeted monoclonal antibody panitumumab. It was specifically evaluated by WiSP Wissenschaftlicher Service Pharma GmbH in the Phase II PURO trial (NCT01374789) as a first-line treatment for patients with locally advanced or metastatic urothelial carcinoma who possess wild-type HRAS. Gemcitabine acts as a nucleoside analog that inhibits DNA synthesis, while cisplatin is a platinum-based agent that induces DNA cross-linking and damage. Panitumumab is a fully human monoclonal antibody that binds to the epidermal growth factor receptor (EGFR), inhibiting downstream signaling pathways involved in cell proliferation and survival. The rationale for this combination is based on the frequent overexpression of EGFR in bladder cancer and the hypothesis that wild-type HRAS status may predict better response to EGFR inhibition.
