Gemcitabine + cisplatin + S-1 is a combination chemotherapy regimen primarily investigated for the treatment of advanced biliary tract cancer. Gemcitabine is a nucleoside analog that inhibits DNA synthesis by incorporating into DNA and inhibiting ribonucleotide reductase, leading to apoptosis in rapidly dividing cells[6]. Cisplatin is a platinum-based chemotherapeutic agent that forms DNA crosslinks and induces apoptosis. S-1 is an oral fluoropyrimidine derivative combining tegafur (a prodrug of 5-fluorouracil), gimeracil (an inhibitor of dihydropyrimidine dehydrogenase), and oteracil (which reduces gastrointestinal toxicity). The triplet regimen aims to enhance antitumor efficacy compared to doublet regimens. Clinical trials have shown promising activity in advanced biliary tract cancer, with ongoing studies evaluating its safety and survival benefit[3][5][8].
