gemcitabine + docetaxel + doxorubicin is a combination chemotherapy regimen evaluated by the Sarcoma Alliance for Research through Collaboration (SARC) for the adjuvant treatment of high-risk uterine leiomyosarcoma (uLMS). The regimen, specifically studied in the SARC005 Phase II clinical trial, involves the sequential administration of gemcitabine and docetaxel followed by doxorubicin after complete surgical resection of the primary tumor. Gemcitabine is a pyrimidine antimetabolite that inhibits DNA synthesis by targeting ribonucleoside reductase and incorporating into DNA. Docetaxel is a taxane that promotes microtubule assembly and inhibits depolymerization, leading to cell cycle arrest. Doxorubicin is an anthracycline that intercalates into DNA and inhibits topoisomerase II, causing double-strand DNA breaks. This multi-agent approach aims to eradicate micrometastatic disease and improve progression-free survival in patients with localized but high-risk uterine leiomyosarcoma.
