The combination of metronomic gemcitabine, doxorubicin, docetaxel, and nivolumab is an investigational chemo-immunotherapy regimen developed by the Sarcoma Oncology Research Center, LLC for the treatment of advanced sarcoma. This regimen, evaluated in the Phase 2 GALLANT trial (NCT04535713), utilizes a metronomic dosing schedule—frequent administration of low-dose chemotherapy—to minimize systemic toxicity while potentially inducing anti-angiogenic effects and enhancing the tumor microenvironment's receptivity to immunotherapy. Gemcitabine acts as a nucleoside analog that inhibits DNA synthesis; doxorubicin is an anthracycline that intercalates DNA and inhibits topoisomerase II; and docetaxel is a taxane that stabilizes microtubules, leading to cell cycle arrest. Nivolumab, a monoclonal antibody targeting the PD-1 receptor, is added to overcome immune evasion. Clinical data from the GALLANT trial has demonstrated a high disease control rate in patients with previously treated advanced sarcoma.
