gemcitabine + fludarabine + melphalan

The gemcitabine-fludarabine-melphalan (GFM) regimen is a reduced-intensity conditioning (RIC) protocol developed by the M.D. Anderson Cancer Center for patients with relapsed or refractory Hodgkin lymphoma undergoing allogeneic stem cell transplantation. This combination therapy integrates gemcitabine, a pyrimidine nucleoside analog, into the established fludarabine and melphalan backbone. The addition of gemcitabine is intended to provide dose-intensified cytoreduction against Hodgkin lymphoma cells while maintaining the immunosuppressive properties required for donor cell engraftment and minimizing the high toxicity associated with traditional myeloablative regimens. It was evaluated in a Phase 2 clinical trial (NCT00385788) showing favorable progression-free and overall survival in high-risk patients.
