This is a combination therapy consisting of four well-known, FDA-approved drugs: gemcitabine, metformin, simvastatin, and digoxin. Each component has a distinct mechanism of action: - Gemcitabine is a nucleoside analog that inhibits DNA synthesis and is primarily used as an antineoplastic agent in various cancers. - Metformin is an oral biguanide antihyperglycemic agent that activates AMP-activated protein kinase (AMPK), reducing hepatic glucose production and improving insulin sensitivity; it also exhibits antiproliferative effects in cancer cells. - Simvastatin is an HMG-CoA reductase inhibitor (statin) that lowers cholesterol biosynthesis and has demonstrated anticancer properties through inhibition of cell proliferation pathways. - Digoxin is a cardiac glycoside that inhibits Na+/K+-ATPase, increasing intracellular calcium in cardiac cells; it also shows potential anticancer activity by modulating cell signaling pathways. Preclinical studies have shown synergistic or additive effects when these agents are combined for the treatment of pancreatic ductal adenocarcinoma (PDAC) and other cancers. The combination targets multiple cellular processes including cell cycle regulation, energy metabolism, apoptosis induction, and inhibition of survival proteins such as BIRC5[1][3][5]. This multi-drug regimen aims to overcome resistance mechanisms seen with monotherapies like gemcitabine alone.
