Overview

Gemcitabine + nab-paclitaxel + hydroxychloroquine is a combination regimen studied primarily for the treatment of pancreatic ductal adenocarcinoma. Gemcitabine is a nucleoside analog that inhibits DNA synthesis, leading to cell death. Nab-paclitaxel is an albumin-bound formulation of paclitaxel that stabilizes microtubules and inhibits cell division. Hydroxychloroquine, traditionally used as an antimalarial and for autoimmune diseases, acts as an autophagy inhibitor by blocking the fusion of autophagosomes with lysosomes. The rationale for this combination is to enhance chemotherapy efficacy by inhibiting autophagy—a survival mechanism in cancer cells—thereby increasing tumor cell death and potentially modulating immune response[1][2][6]. This regimen has been evaluated in phase 1 and 2 clinical trials for resectable and metastatic pancreatic cancer[1][3][4].
