This is a combination regimen consisting of three agents: - **Gemcitabine** is a nucleoside analog used as an antimetabolite chemotherapy agent. It inhibits DNA synthesis by incorporation into DNA and inhibition of ribonucleotide reductase, leading to cell death. - **Oxaliplatin** is a platinum-based chemotherapeutic that forms crosslinks in DNA, inhibiting replication and transcription, ultimately causing apoptosis. - **Photosan** (commonly referring to "Photofrin", generic name porfimer sodium) is a photosensitizer used in photodynamic therapy. After intravenous administration, it accumulates in tumor tissue; upon activation by specific wavelength light (usually laser), it generates reactive oxygen species that cause local cytotoxicity and tumor cell death. This combination would be considered investigational or experimental for most cancers unless part of a clinical trial. Each component has established use in oncology—gemcitabine and oxaliplatin are standard chemotherapies for various solid tumors; Photofrin (porfimer sodium) is approved for photodynamic therapy in esophageal cancer, endobronchial non-small-cell lung cancer, and high-grade dysplasia in Barrett’s esophagus[1][3]. The rationale for combining these agents would be to leverage both systemic cytotoxic effects (from gemcitabine/oxaliplatin) with localized photodynamic destruction from Photosan.
