gemcitabine + vinorelbine + trastuzumab is a combination chemotherapy and targeted therapy regimen used primarily in the treatment of HER2-positive metastatic breast cancer. The regimen combines two cytotoxic agents—gemcitabine (a nucleoside analog that inhibits DNA synthesis) and vinorelbine (a vinca alkaloid that disrupts microtubule assembly)—with trastuzumab, a monoclonal antibody that targets the human epidermal growth factor receptor 2 (HER2). This triplet combination is often evaluated in patients who have progressed on prior taxane- or anthracycline-based therapies, aiming to provide synergistic anti-tumor activity by targeting multiple pathways involved in cell proliferation and survival. Gemcitabine acts as a prodrug that, once phosphorylated, inhibits DNA polymerase and ribonucleoside reductase, while vinorelbine binds to tubulin to prevent mitotic spindle formation. Trastuzumab binds to the extracellular domain of HER2, inhibiting downstream signaling pathways such as PI3K/Akt and promoting antibody-dependent cellular cytotoxicity (ADCC).
