**Gemigliptin + glimepiride** is an oral fixed-dose antidiabetic combination developed by LG Life Sciences and subsequently associated with LG Chem for adults with type 2 diabetes mellitus inadequately controlled with glimepiride alone. Gemigliptin is a selective, reversible dipeptidyl peptidase-4 inhibitor that prolongs endogenous incretin activity, increasing glucose-dependent insulin secretion and suppressing glucagon secretion. Glimepiride is a sulfonylurea that binds the sulfonylurea receptor 1 subunit of pancreatic beta-cell ATP-sensitive potassium channels, promoting channel closure, membrane depolarization, calcium influx, and insulin release. A registered Phase III study evaluated gemigliptin 50 mg once daily added to ongoing glimepiride as a fixed-dose combination in type 2 diabetes mellitus.
