This is a **combination of three antibiotics**—gentamicin, cefazolin, and vancomycin—administered together for synergistic or broadened antimicrobial therapy, particularly in severe or high-risk infections. Each component has a distinct mechanism of action: - **Gentamicin** is an aminoglycoside antibiotic that binds to the 30S ribosomal subunit, inhibiting protein synthesis and exerting a bactericidal effect. - **Cefazolin** is a first-generation cephalosporin (β-lactam) antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins, also resulting in bacterial cell lysis. - **Vancomycin** is a glycopeptide antibiotic that inhibits cell wall synthesis by binding to D-alanyl-D-alanine termini of cell wall precursor units. This combination may be used for surgical prophylaxis, severe infections, or in cases of resistant or mixed-organism infections, such as those caused by MRSA or in patients with severe β-lactam allergies. There is evidence that combining cefazolin and gentamicin can be superior to vancomycin alone in certain hemodialysis patient infections[2]. Additionally, cefazolin and vancomycin given together can improve antibacterial kill rates against MRSA compared to either agent alone[3][1]. Triple combination use is less common but may be reserved for complex, high-risk scenarios where broad-spectrum and synergistic coverage is critical.
