Gentamicin + narrow spectrum betalactam is a combination antibiotic regimen consisting of gentamicin, an aminoglycoside, and a narrow-spectrum beta-lactam antibiotic (such as penicillin or first-generation cephalosporins). Gentamicin acts by binding to the 30S subunit of bacterial ribosomes, inhibiting protein synthesis and causing bacterial cell death. Narrow-spectrum beta-lactams inhibit bacterial cell wall synthesis by binding to penicillin-binding proteins. This combination is used empirically for severe infections such as sepsis, endocarditis, intra-abdominal infections, and complicated urinary tract infections. The rationale for combining these agents is their synergistic bactericidal activity against certain Gram-positive and Gram-negative bacteria; the beta-lactam increases cell wall permeability allowing greater gentamicin uptake[2][4][5][6]. However, this regimen carries increased risk of nephrotoxicity compared to monotherapy.
