Overview

Gentamicin + polymyxin + probiotics is a combination therapeutic regimen designed for the selective digestive decolonization (SDD) of carbapenem-resistant Enterobacteriaceae (CRE). Developed by the First Affiliated Hospital, Zhejiang University School of Medicine, the regimen typically involves the oral administration of gentamicin and a polymyxin (such as polymyxin E/colistin) to eliminate resistant Gram-negative bacteria from the intestinal reservoir. Gentamicin acts by binding to the bacterial 30S ribosomal subunit to inhibit protein synthesis, while polymyxins disrupt the bacterial outer membrane by binding to lipopolysaccharides (LPS). The inclusion of probiotics is intended to mitigate the disruption of the normal gut microbiota caused by the antibiotics, promoting the restoration of a healthy microbial environment and preventing the recurrence of CRE colonization.
