The combination of a gentamicin-collagen sponge and levofloxacin is used as an adjunctive therapy for the treatment of infected skin ulcers, particularly in diabetic foot infections. The gentamicin-collagen sponge is a topical device made from bovine collagen impregnated with the aminoglycoside antibiotic gentamicin. When applied to wounds, the collagen matrix gradually breaks down, releasing high local concentrations of gentamicin directly into the ulcer site with minimal systemic absorption. Gentamicin acts by binding to bacterial 16S rRNA in the 30S ribosomal subunit, disrupting protein synthesis and causing bacterial cell death. Levofloxacin is a systemic fluoroquinolone antibiotic that inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for DNA replication and cell division, leading to bactericidal activity[6][7]. This combination aims to enhance infection control by providing both local (gentamicin) and systemic (levofloxacin) antibacterial effects. Clinical studies have evaluated this approach primarily in moderate or mild diabetic foot infections; results suggest potential benefits in pathogen eradication rates and clinical cure when compared to standard care alone[1][2][3].
