GHRH + acipimox is a combination therapy involving growth hormone-releasing hormone (GHRH), a peptide that stimulates the secretion of growth hormone (GH) from the pituitary gland, and acipimox, an antilipolytic small molecule drug that reduces plasma free fatty acid (FFA) levels. Acipimox is a nicotinic acid analog primarily used to lower FFA concentrations by inhibiting lipolysis in adipose tissue. The rationale for combining these agents is based on evidence that elevated FFAs can suppress GH secretion, particularly in conditions such as obesity or metabolic disorders. By reducing FFA levels with acipimox, the GH response to GHRH stimulation can be significantly enhanced. This combination has been studied in both animal models and humans for its potential to improve linear body growth and metabolic outcomes beyond what is achieved with GHRH or GH alone[1][3][6]. In clinical studies, this approach has shown promise in overcoming blunted GH responses seen in obesity and type 1 diabetes[1][6], as well as enhancing linear growth while mitigating some adverse metabolic effects associated with GH therapy[3][5].
