Overview

This regimen is a combination of four direct-acting antivirals (**glecaprevir**, **pibrentasvir**, **sofosbuvir**) and one nucleoside analog (**ribavirin**) used primarily as a retreatment strategy for chronic hepatitis C virus (HCV) infection in patients who have failed prior therapy with glecaprevir/pibrentasvir. - **Glecaprevir** is an NS3/4A protease inhibitor that blocks viral polyprotein processing. - **Pibrentasvir** is an NS5A inhibitor that impairs viral replication and assembly. - **Sofosbuvir** is a nucleotide analog NS5B polymerase inhibitor that blocks HCV RNA synthesis. - **Ribavirin** is a nucleoside analog that enhances viral mutagenesis and immune-mediated clearance. This four-drug combination is recommended for 16 weeks in patients with or without compensated cirrhosis who failed prior glecaprevir/pibrentasvir therapy. The regimen was supported by the MAGELLAN-3 study, showing high sustained virologic response rates across genotypes[2].
