glimepiride + fenofibrate

Glimepiride + fenofibrate is a combination of two small molecule drugs used together, though not as a fixed-dose commercial product. Glimepiride is a sulfonylurea antidiabetic agent that lowers blood glucose by stimulating insulin release from pancreatic beta cells and increasing peripheral tissue sensitivity to insulin. Fenofibrate is a lipid-lowering agent that acts as an agonist of peroxisome proliferator activated receptor alpha (PPARα), leading to increased lipolysis and elimination of triglyceride-rich particles, reduction in apoprotein C-III, and overall improvement in lipid profiles. This combination may be used off-label in patients with type 2 diabetes mellitus who also have dyslipidemia or hypertriglyceridemia, but it carries an increased risk of hypoglycemia due to pharmacokinetic interactions that raise glimepiride exposure and enhance its glucose-lowering effect[1][3][4][7]. There are no known branded or approved fixed-dose combinations containing both agents.
