Novorapid + Amaryl is a combination drug regimen consisting of the rapid-acting recombinant insulin analogue **insulin aspart** (NovoRapid) and the third-generation sulfonylurea **glimepiride** (Amaryl). This regimen is primarily used preprandially in patients with type 2 diabetes mellitus who have failed to achieve adequate glycemic control on maximal oral antidiabetic therapy, including metformin. Insulin aspart acts as an agonist at the insulin receptor to facilitate glucose uptake and suppress hepatic glucose production. Glimepiride stimulates endogenous insulin secretion by binding to the sulfonylurea receptor (SUR1) subunit of ATP-sensitive potassium channels in pancreatic beta cells, leading to channel closure and subsequent cell depolarization. The combination was evaluated in the LANN (Lantus, Amaryl, Novorapid, Novomix) randomized controlled trial to determine if combining a secretagogue with short-acting insulin could achieve comparable glycemic control with less weight gain than conventional basal or premixed insulin regimens.
