This is an oral **combination drug** made up of five antidiabetic agents: **glimepiride**, **metformin**, **sitagliptin**, **gliclazide**, and **glibenclamide**. All five ingredients lower blood glucose through complementary mechanisms for the treatment of type 2 diabetes mellitus. - **Metformin** (a biguanide) reduces hepatic glucose production, decreases intestinal glucose absorption, and increases insulin sensitivity. - **Glimepiride**, **gliclazide**, and **glibenclamide** are sulfonylureas; they stimulate pancreatic β-cells to secrete insulin, thereby lowering blood glucose. - **Sitagliptin** is a dipeptidyl peptidase-4 (DPP-4) inhibitor, which increases incretin levels to enhance glucose-dependent insulin release and suppress glucagon secretion. Combination of multiple sulfonylureas with metformin and sitagliptin is not standard and may pose heightened risk of hypoglycemia, but theoretically results in multi-targeted glucose control. No unique brand or formulation is reported for this five-drug combination; such use would be non-standard and not widely approved or marketed. All components are well-established drugs individually and in various dual or triple combinations[1][5][7].
