This is an investigational combination therapy protocol developed by Sohag University for the treatment of acute aluminum phosphide (ALP) poisoning, a condition with no known specific antidote and a high mortality rate. The protocol integrates three distinct therapeutic approaches: Glucose-Insulin-Potassium (GIK) infusion, intravenous hydrocortisone, and oral sevelamer. GIK therapy, also known as hyperinsulinemia-euglycemia therapy, is intended to shift myocardial metabolism toward carbohydrates, thereby improving cardiac function and systemic vascular resistance while correcting metabolic acidosis. Hydrocortisone is administered to address potential adrenal insufficiency and stabilize cell membranes in the context of cardiogenic shock. Sevelamer, a phosphate binder typically used in chronic kidney disease, is repurposed in this protocol to act as an oral antidote by directly binding and neutralizing phosphine gas within the gastrointestinal tract. This multi-modal approach aims to reduce the high mortality associated with ALP-induced cardiovascular collapse and metabolic failure.
