GIK-Xijing Hospital-myocardial injury refers to a specific modified glucose-insulin-potassium (GIK) regimen developed and evaluated by Xijing Hospital (Fourth Military Medical University) for myocardial protection in patients undergoing cardiac surgery with cardiopulmonary bypass. The formulation consists of 200 g/L glucose, 66.7 U/L insulin, and 80 mmol/L potassium chloride (KCl), administered as a continuous intravenous infusion starting 10 minutes before anesthesia and continuing for 12.5 hours. This metabolic intervention is designed to shift myocardial metabolism from fatty acid oxidation to the more oxygen-efficient glucose oxidation and to activate insulin-mediated pro-survival signaling pathways (such as Akt/PI3K). A large-scale randomized, double-blind, placebo-controlled trial (NCT01516138) involving 930 patients demonstrated that this specific GIK protocol significantly reduces the incidence of in-hospital major adverse cardiac events and enhances the recovery of cardiac function following surgery.
