Overview
GnRH agonist + HRT refers to the combination of a gonadotropin-releasing hormone (GnRH) agonist with hormone replacement therapy (HRT), typically involving low-dose estrogen and/or progestin. GnRH agonists are synthetic analogs of the natural hypothalamic peptide that initially stimulate and then downregulate pituitary gonadotropin secretion, leading to decreased ovarian production of estrogen and progesterone. This results in a hypoestrogenic state useful for treating conditions like endometriosis, uterine fibroids (myomas), and for reproductive cycle control in assisted reproduction. However, prolonged hypoestrogenism can cause significant side effects such as bone mineral density loss and vasomotor symptoms. The addition of HRT ("add-back" therapy) mitigates these adverse effects by supplementing small amounts of sex hormones without compromising the efficacy of the underlying condition treatment[1][2][3][4][5][7]. This regimen is also used in fertility preservation protocols.
