Overview

**Golimumab + thiopurines** is a combination therapy used for treating chronic immune-mediated inflammatory disorders, primarily *ulcerative colitis* and other forms of *inflammatory bowel disease (IBD)*. Golimumab is a fully human monoclonal antibody targeting tumor necrosis factor (TNF), a key cytokine in inflammatory pathways. Thiopurines (including azathioprine and 6-mercaptopurine) are oral immunosuppressive agents that inhibit purine synthesis, thus dampening immune responses. The combination is primarily investigated and used to improve therapeutic outcomes—such as remission rates and antibody formation prevention—compared to monotherapy with either agent. Clinical studies and meta-analyses suggest that combination therapy may increase remission rates but also raises infection and malignancy (notably lymphoproliferative disease) risk[1][3]. Mechanistic rationale centers around reducing anti-drug antibody formation and achieving synergistic immunomodulation.
