The core of the regimen consists of goserelin (a GnRH agonist), abiraterone acetate (a CYP17 inhibitor), and prednisone (a corticosteroid). A distinguishing feature of the "type-2" arm is the integration of a targeted therapy selected based on the patient's specific genetic mutations identified through Next-Generation Sequencing (NGS). If no actionable mutations are found, the patient receives the triple combination of goserelin, abiraterone, and prednisone. This approach aims to personalize treatment by matching therapeutic agents to the genomic profile of the tumor in patients who do not fit standard luminal or neuroendocrine subtypes.
