Granulocyte transfusion is an allogeneic cell therapy consisting of concentrated neutrophils collected from healthy donors. In the RING Study (Resolving Infection in Neutropenia with Granulocytes), sponsored by Carelon Research, donors were mobilized with granulocyte colony-stimulating factor (G-CSF) and dexamethasone to maximize the yield of cells. The resulting high-dose granulocyte product (targeted at ≥4 x 10^10 cells per transfusion) is administered intravenously to patients with severe neutropenia (absolute neutrophil count <500/mm³) who have developed life-threatening bacterial or fungal infections. The therapy aims to provide immediate, functional phagocytes to bridge the gap until the patient's endogenous white blood cell count recovers, thereby assisting in the clearance of pathogens.
