Grape seed polyphenolic extract + resveratrol is an unbranded oral nutraceutical combination evaluated in a completed Phase 1 Johns Hopkins University study in people with mild cognitive impairment and prediabetes or type 2 diabetes mellitus, including individuals at risk of Alzheimer’s disease. The registered intervention combines a grape seed-derived polyphenol extract with resveratrol. Its proposed neuroprotective activity is broad rather than attributable to a single validated drug target: grape-derived polyphenols may modulate oxidative stress, inflammatory signaling, insulin-resistance-associated brain dysfunction, synaptic plasticity, and amyloid-beta and tau pathology. The clinical trial record describes preclinical observations of improved cognition and long-term potentiation and reduced amyloid and tau burden in mouse models, but this combination is not an approved pharmaceutical treatment for Alzheimer’s disease.
