The combination of H2-receptor antagonists (H2RAs) and proton pump inhibitors (PPIs) is used to enhance gastric acid suppression beyond what can be achieved with either medication alone. H2RAs work by blocking histamine-2 receptors on gastric parietal cells, preventing histamine from binding and reducing basal gastric acid secretion. PPIs inhibit the H+/K+-ATP enzymes of gastric parietal cells, resulting in continuous and robust inhibition of gastric acid secretion. When administered concomitantly, these medications can provide more comprehensive acid control than monotherapy, particularly for conditions like gastroesophageal reflux disease (GERD) and duodenal ulcers.
