Drug intelligence / Profile preview
Half-strength Darrow's solution in 5% dextrose (HSD/5%D) is a specialized intravenous fluid formulation designed for the management of shock and severe dehydration, particularly in pediatric patients with severe acute malnutrition (SAM). Recommended by the World Health Organization (WHO), this hypotonic solution contains essential electrolytes—sodium chloride, potassium chloride, and sodium lactate—combined with 5% dextrose (glucose). Its composition is tailored to the unique physiological needs of malnourished children, who often present with elevated total body sodium and significant potassium deficits. The inclusion of dextrose provides a source of energy and helps prevent hypoglycemia during fluid resuscitation. HSD/5%D acts by restoring intravascular volume and correcting critical fluid and electrolyte imbalances. It has been evaluated in clinical trials, such as the Phase II 'Fluid Expansion in Severe Malnutrition' trial sponsored by the University of Oxford, comparing its efficacy against other resuscitation fluids like Ringer's lactate and albumin.
