Overview

Haloperidol + lorazepam is a combination of two pharmaceutical agents used primarily for the rapid tranquilization and management of acute agitation, particularly in psychiatric emergencies. Haloperidol is a high-potency first-generation (typical) antipsychotic that acts mainly as an antagonist at dopamine D2 receptors, reducing psychotic symptoms and agitation. Lorazepam is a short-acting benzodiazepine that acts as a positive allosteric modulator at the GABA(A) receptor, producing sedative, anxiolytic, muscle relaxant, and anticonvulsant effects. The combination is often used when monotherapy with either agent alone does not provide sufficient control of severe agitation or delirium. Clinical studies suggest that while the combination can be effective for controlling acute agitation or persistent delirium (such as in end-of-life care), it may not always offer significant advantages over haloperidol alone in terms of efficacy but may increase risks such as sedation or hypotension[1][2][3][4][9].
