A specialized hematopoietic stem cell transplantation (HSCT) strategy developed by the Case Comprehensive Cancer Center. This approach involves the co-infusion of an unmodified haplo-identical (half-matched) donor graft and a single umbilical cord blood (UCB) unit. The primary rationale is to utilize the haplo-identical graft to ensure rapid neutrophil and platelet engraftment, thereby minimizing the early infectious risks associated with the delayed engraftment typically seen in cord blood transplants. Simultaneously, the cord blood unit is intended to provide durable, long-term hematopoiesis and a potent graft-versus-leukemia (GVL) effect. This dual-source strategy is designed for adult patients with high-risk hematologic malignancies who lack a readily available HLA-matched sibling or unrelated donor.
