Overview

Haploidentical donor CD34+ cells refer to hematopoietic stem and progenitor cells (HSPCs) that are positively selected for the surface marker CD34 from a related donor who shares only one HLA haplotype with the recipient (i.e., is "haploidentical"). These cell products are used in allogeneic hematopoietic stem cell transplantation (allo-HSCT), particularly when matched sibling or unrelated donors are unavailable. The positive selection of CD34+ cells is typically performed using immunomagnetic separation technologies such as CliniMACS. This approach allows for T-cell depletion, which reduces the risk of graft-versus-host disease (GvHD) while enabling engraftment and immune reconstitution. Haploidentical transplants with megadose or high-dose purified CD34+ cells have been shown to achieve successful engraftment and favorable outcomes in both malignant (e.g., acute myeloid leukemia) and nonmalignant diseases[1][6][9]. The main mechanism involves reconstituting the recipient's hematopoiesis by providing multipotent progenitor/stem cells capable of generating all blood lineages.
